PDN Volunteers - Associate Member Form

Many of the projects we accomplish require vetting of our volunteers to one degree or another.  The military caseworkers we work with expect us to exercise due diligence in screening some volunteers who may serve in a "caregiver" capacity.  (See Code of Ethics at end of page)

For most projects, only your verifiable contact information is needed. We also do not want to disturb you at work if you're unable to be contacted there. Do not list your work phone if that is the case. Some other fields are required however. The information you provide is secure and not available to third parties or other volunteers.
